That\u2019s something you might really <em class=\"ly\">want<\/em>. In my case I need a \u201cblood thinner\u201d (as they are called) to curb my blood\u2019s overenthusiastic zeal to clot for no particular reason, due to a genetic disorder called \u201cFactor V Leiden\u201d. Twenty-five years ago I had a DVT (Deep Venous Thrombosis) followed by a second PE (Pulmonary Embolism) \u2014 a clot that breaks loose and gets caught in the peerless filter of one\u2019s lung; unfortunately, whatever part of said lung is downstream of the plug <em class=\"ly\">dies<\/em>. Unpleasantly. My first PE was misdiagnosed as \u201cpleurisy\u201d, but once you have experienced one, there\u2019s no mistaking it. Trust me, you don\u2019t want to, unless you find unprotected swordfighting a bracing pastime.<\/p>\n<p id=\"821c\" class=\"pw-post-body-paragraph la lb fq lc b ld le lf lg lh li lj lk ll lm ln lo lp lq lr ls lt lu lv lw lx fj bk\" data-selectable-paragraph=\"\">So, for the past 17 years I\u2019ve been taking a daily dose of Coumadin, a cheap drug based on <a class=\"af re\" href=\"https:\/\/en.wikipedia.org\/wiki\/Warfarin\" target=\"_blank\" rel=\"noopener ugc nofollow\"><strong class=\"lc fr\">Warfarin<\/strong><\/a>, a.k.a. <em>rat poison<\/em>. Seriously, it\u2019s cheap because untold tons of the stuff are churned out every year to feed to unwanted rats and mice (and, if you\u2019re careless, other small animals). They die of dehydration and uncontrolled bleeding, if you really want to know. So there\u2019s considerable motivation to monitor <em class=\"ly\">how<\/em> well the Coumadin is working. The only way to do this that I know of (you med-tech jocks out there, see if you can\u2019t make a phone app to do this non-invasively!) is to stick a needle in a vein and take a blood sample to send to the lab to check your INR (International Normalized Ratio \u2014 informative, eh?) or PT (Prothrombin Time). In Europe, it is more common to use a finger-prick and measure your INR on the spot from a glass capillary full of the red stuff. Basically, the higher your INR, the longer it takes for your blood to clot \u2014 the more \u201cthinned\u201d it is. For me, the best INR is between 2 and 3. The point is, you want to know that it\u2019s <em class=\"ly\">working<\/em> (no more PEs) and also that it\u2019s not working <em class=\"ly\">too well<\/em> (remember the rats). When your INR is too high, you can cut back your daily dose a little; if too low, take a little more. It works.<\/p>\n<p id=\"29cf\" class=\"pw-post-body-paragraph la lb fq lc b ld le lf lg lh li lj lk ll lm ln lo lp lq lr ls lt lu lv lw lx fj bk\" data-selectable-paragraph=\"\">Enough about me. Back to the Pharmas. They discovered that whereas Warfarin works by preventing the liver from processing vitamin K to make \u201cbiologically active forms of the calcium-dependent clotting factors II, VII, IX and X, as well as the regulatory factors protein C, protein S, and protein Z\u201d [I\u2019m copying from the Wikipedia article; I don\u2019t know what they\u2019re talking about either], the way the new alternatives \u201cwork in your body\u00a0is different from the way warfarin works. They affect a different part of the clotting process. This difference often makes these newer drugs convenient to use.\u201d [Now I\u2019m copying from a <a class=\"af re\" href=\"http:\/\/www.healthline.com\/health\/dvt\/warfarin-alternatives#overview1\" target=\"_blank\" rel=\"noopener ugc nofollow\"><em class=\"ly\">Healthline<\/em><\/a> article that was the most informative source I could Google easily.] That article lists 7 examples, and how they are administered, but offers no technical details on <em class=\"ly\">which<\/em> parts of the clotting process they affect, or <em class=\"ly\">how<\/em>.<\/p>\n<p id=\"f2fb\" class=\"pw-post-body-paragraph la lb fq lc b ld le lf lg lh li lj lk ll lm ln lo lp lq lr ls lt lu lv lw lx fj bk\" data-selectable-paragraph=\"\">Well, fine, I guess; who wants to know all those technical details (other than me, of course). One can see why <em class=\"ly\">Healthline<\/em> might balk at telling the whole long technical story to impatient readers. But wait\u2026<\/p>\n<p id=\"90c0\" class=\"pw-post-body-paragraph la lb fq lc b ld le lf lg lh li lj lk ll lm ln lo lp lq lr ls lt lu lv lw lx fj bk\" data-selectable-paragraph=\"\">They (and all the other analogous sites I\u2019ve visited, <em class=\"ly\">and<\/em> all the ads we are now seeing on TV) go on to list Advantages and Disadvantages (over Warfarin), and there is a glaring omission in every case \u2014 while \u201cYou need fewer tests during treatment\u201d is always listed under Advantages, there is never any mention under Disadvantages of the following rather significant fact, which I believe applies to <em class=\"ly\">all<\/em> these new drugs (though it\u2019s not easy to find out for sure):<\/p>\n<h2 id=\"771b\" class=\"ahh ahi fq bf ahj ahk ahl ahm pg ahn aho ahp pj ll ahq ot ou lp ahr ov ow lt ahs ox oy aht bk\" data-selectable-paragraph=\"\"><strong class=\"al\">There IS NO reliable clinical test to see how well it\u2019s working.<\/strong><\/h2>\n<p id=\"8fdb\" class=\"pw-post-body-paragraph la lb fq lc b ld ahu lf lg lh ahv lj lk ll ahw ln lo lp ahx lr ls lt ahy lv lw lx fj bk\" data-selectable-paragraph=\"\">(Well, strictly speaking, there is: wait to see if you have a PE \u2014 if you do, it\u2019s not working; and\/or cut yourself to see if you ever stop bleeding \u2014 if not, it\u2019s working too well.)<\/p>\n<p id=\"4774\" class=\"pw-post-body-paragraph la lb fq lc b ld le lf lg lh li lj lk ll lm ln lo lp lq lr ls lt lu lv lw lx fj bk\" data-selectable-paragraph=\"\">This is probably quite convenient for your GP, whose responsibility ends with the prescription. It saves your health care provider about $17\/month (the typical cost of INR tests) and it rather dramatically obscures the cause of death in those (doubtless rare) cases where the stuff works too well or not well enough. There aren\u2019t even alternative dose recommendations; one size fits all!<\/p>\n<p id=\"dd41\" class=\"pw-post-body-paragraph la lb fq lc b ld le lf lg lh li lj lk ll lm ln lo lp lq lr ls lt lu lv lw lx fj bk\" data-selectable-paragraph=\"\">Most of all, of course, it suits the Pharmas, since these drugs are quite <em class=\"ly\">expensive<\/em> \u2014 especially compared with good ol\u2019 <em>rat poison<\/em>. Finally they are able to make a decent [obscene] profit off us Leiden-factor folks! This cost will usually be split between your health care provider and you personally.<\/p>\n<p id=\"b7f6\" class=\"pw-post-body-paragraph la lb fq lc b ld le lf lg lh li lj lk ll lm ln lo lp lq lr ls lt lu lv lw lx fj bk\" data-selectable-paragraph=\"\">Oh yes, there is one other Disadvantage: with (I understand) 1 or 2 exceptions, there is no way to \u201cturn off\u201d the new drugs if it becomes painfully obvious that they are working <em class=\"ly\">too<\/em> well. (This would presumably be signaled by uncontrolled bleeding from various orifices. <em class=\"ly\">Hi, Mr. Rat<\/em>!)<\/p>\n<p id=\"6066\" class=\"pw-post-body-paragraph la lb fq lc b ld le lf lg lh li lj lk ll lm ln lo lp lq lr ls lt lu lv lw lx fj bk\" data-selectable-paragraph=\"\">So\u2026 nothing new here, really, just Pharmas doing their thing. Except somehow they have managed to enlist government agencies and GPs in their campaign to convince us that<\/p>\n<h1 id=\"f161\" class=\"ahz ahi fq bf ahj pe aia pf pg ph aib pi pj pk aic pl pm pn aid po pp pq aie pr ps aif bk\" data-selectable-paragraph=\"\">Ignorance is the best strategy!<\/h1>\n<p id=\"8a73\" class=\"pw-post-body-paragraph la lb fq lc b ld ahu lf lg lh ahv lj lk ll ahw ln lo lp ahx lr ls lt ahy lv lw lx fj bk\" data-selectable-paragraph=\"\">That is certainly a revolutionary new concept in medicine.
